Competitive Pay Based on Experience
Shift Differential Pay (at participating locations)

Essential Functions of Wound Nurse (RN or LPN):
Reasonable accommodations may be made to enable individuals with

Provide personal care to residents in a manner conductive to their
safety and comfort consistent with Company Clinical Policies and
Procedures as well as state/federal guidelines and
-regulations.
Assesses, treats, and cares for patients with wounds such as
pressure ulcers, ostomies, skin breakdowns or other wounds.
Treatment tasks include wound debridement, cleaning, bandaging, and
working with the care team to determine if other treatments are
necessary or creating a plan to boost healing opportunity or
preventing wound re-emergence.
Assess residents on admission and -document appropriately as
condition changes.
Ability to adequately assess and reassess residents' pain, utilize
appropriate pain management techniques, and education resident and
family on pain -management.
Interpret test results and take appropriate action on -results.
Promote and support the greatest possible degree of independence
for -residents.
Advise oncoming staff of resident's status at beginning and end of
shifts and properly documenting.
Contribute knowledge of residents' conditions and document
observations as -required.
Promote a safe and clean hazard-free environment for residents,
family members, and staff -members
-Assure equipment is clean and functional. Report any malfunctions
to -supervisor.
-Ability to function as a team leader and role -mode
-Abide by the Company Clinical policies and procedures for Abuse
-Prevention.
-Notify physician of changes in resident's condition and follow
through until appropriate action is -taken.
-Accurately and promptly implement physician -orders.
Follow through, as needed, on information given by shift report,
resident or -family regarding resident -concerns.
-Implement plan of care for the resident based on assessments and
goals as -established by the interdisciplinary care -team.
Ability to react decisively and quickly in emergency
-situations.
Perform other task and duties as -assigned.
-Qualifications of -Wound Nurse (RN or LPN):
Current LPN or RN State issued license is required
WOCN certification in good standing is required
1 year nursing experience in a long-term care facility is
preferred
Ability to communicate effectively with residents and -their family
members, and at all levels of the organization
Ability to react decisively and quickly in emergency situations
Ethical practice
Ability to communicate clearly
Strong Professionalism
Ability to maintain confidentiality
Ability to read, write, and speak the English -language
Valid CPR -Certification
