Job Description:

Start date May 18th or as soon as credentialed

Month to Month with the option to extend
(Jefferson County, Franklin County, St. Louis County)

Position Highlights:
Full-time only (40 hours/week) Monday Friday
8 am-5 pm with a 1-hour lunch
One-hour lunch break included
Equipment will be mailed out and provided by the client
70% telehealth, 30% Home health, will schedule home health visits within a 30-mile radius of the provided address
MUST BE COMFORTABLE SEEING PEDIATRICS AND ADULTS
Schedules are released 2 3 weeks in advance, so the NP can get familiar with the patients.
Paid for a full day regardless of patient volume (providers must enter availability into Salesforce to be paid for the full day)
1 3 hours of paid orientation, all additional ongoing education modules are expected to be completed during downtime within working hours
Must be enrolled/Credentialed in Medicard (please highlight in submission and confirm with provider)

Role Overview:
For telehealth, the NP should expect 12-14 per day on their schedule. If only doing in-home visits, they should expect 6-7. If it is a mix, it usually falls around 10
Perform annual risk assessments and chronic condition reviews
Assessment-only role no treatment or prescribing required
Escalate urgent findings to the patient s PCP

Must have an active MO Medical license
